2 Medi-Cal requires prior approval from your dentist to receive crowns or dentures. Medi-Cal will cover crowns and full or partial dentures every five years or as medically necessary and if approved by a Medi-Cal dental provider. Medi-Cal may cover crowns on back teeth in some cases. Blue Shield Medicare + Medi-Cal Plan (HMO D-SNP) will help coordinate your Denti-Cal services.
3 Benefit is not covered by Medicare. When you use an in-network provider, you pay $0 for eyeglass frames priced up to $500 every two years. Each year, you also pay $0 for eyeglass lenses, or a $500 contact lens allowance every year, but not both. You are responsible for any amount above the applicable benefit limits.
4 $240 quarterly allowance for food and produce and over-the-counter (OTC) items. This allowance cannot be rolled over from quarter to quarter. The Food and Produce benefit mentioned is a Special Supplemental Benefit for the Chronically Ill (SSBCI). Not all plan members will qualify. Eligibility depends on meeting the definition of a "chronically ill enrollee." Qualifying chronic conditions include but are not limited to: cardiovascular disorders; chronic heart failure; diabetes mellitus; overweight, obesity, and metabolic syndrome; and chronic lung disorders. Please refer to the plan’s member handbook for a complete list of qualifying chronic conditions and eligibility requirements.
